Here is a master checklist for our travelers to reference the requirements we need before an assignment begins. Requirements vary per job specialty, so don’t worry if you don’t have them all!
Medical Documents
Physical/health statement – last 12 months
TB skin test or TB QuantiFERON/Gold – last 12 months or chest X-ray from last five years
Hep B vaccines/Titers – proof of three shots *
MMR vaccines/Titers – proof of three shots *
* Only for specific settings (hospital, clinic, etc.); not required by all job specialties.
